Abstract: 

Basic life support (BLS) following by Advanced cardiac life support (ACLS) is intended to rescue the patients with acute circulatory or respiratory failure or both. The most important determinant of short and long-term neurologically intact survival is the interval from the onset of the cardiac or respiratory onset to restoration of effective spontaneous functions of these vital activities.It is commonly accepted that every physician, regardless of specialty, should be able to  perform CPR. It must be also emphasized that CPR, almost invariably, necessitates a rapid interventional follow-care with ACLS procedure.Without well-performed basic life support, advanced cardiac life support is of no remark-: able benefit, BLS and ACLS are processes that must be performed step by step and with respect to the patient's condition.

Abstract: 

Various dermatoglyphic parameters like finger print pattern, atd angle, absolute ridge count & ab, bc, cd, and ad ridge counts were observed in 150 cases of congenital cardiac disease, comprising of 72 cases of Ventricular Septal Defects (VSD), 60 cases of Atrial Septal Defects (ASD), 9 cases of Coarctation of Aorta (COA) & 9 cases of Tetralogy of Fallot’s (TOF). Same dermatoglyphic parameters were also studied in 300 controls and statistical comparison of cases and controls was done. In our study it was observed that the congenital cardiac disease cases exhibited preponderance of whorls (55.8%) with decrease in loop pattern (36.2%) as compared to those of controls and the difference was highly significant (P<0.001). The difference in the mean total finger ridge count (TFRC) of the controls and of the cases of Congenital Cardiac Diseases (CCD) was found to be highly significant (P<0.001), while the mean atd angle in the cases of Congenital Cardiac Disease (CCD) was widen up and was statistically significant too. The mean ab, the mean bc ridge, the mean cd ridge and the mean ad ridge counts were also higher in the various type of CCD as compared to that controls and on statistical comparison, the difference was found to be highly significant.

Abstract: 

Introduction. This fascinating disorder is also known as Sinus Histiocytosis. It is very rare, probably less than 1000 cases reported in the literature. R.D.D affects lymph nodes, although extranodal involvement has been reported such as: Centaral nervous system (CN.S). All of the cases of R.D.D with CN.S involvement without other sites involvement are 11 cases. Methods. The case is a male patient, 26y/o, with a history of head trauma 2 years ago that has been resulted to unconsciousness. He has been referred to a hospital and brain CT scan and brain M.R.I has been done for him. In M.R.I a brain tumor with left orbit and sphenoidal sinus involvement and compression effect on frontal lobe has been seen. A neurosurgeon operated him and the pathological report was Rosai-Dorfman disease. Then he was referred to radiation therapy department.Results. Treatment approach in CN.S involvement cases consists of surgery, radiotherapy and chemotherapy. Discussion. Over all, response has been inferior to those expected with malignant hematopoietic neoplasms.

Abstract: 

هدف: این مطالعه مروری با هدف بررسی تاثیرات، چالش ها و راهکارهای ارایه مراقبت معنوی در بیماران قلبی انجام شد. زمینه. مدیریت بیماران قلبی پیچیده است و سیر بیماری قابل پیش بینی نیست. نیازهای افراد مبتلا به بیماری قلبی متنوع است. شواهد موجود سطح بالایی از نیازهای معنوی بیماران و تمایل به دریافت مراقبت معنوی را تایید می کنند. روش کار: در این مطالعه برای جمع آوری داده ها از پایگاه های داده ای علمی و موتورهای جست وجو، نظیر Google Scholar، SID، Scopus، PubMed وWeb of Science برای یافتن متون منتشرشده از ابتدای سال 2000 تا سال 2020 استفاده شد. کلیدواژه های مورد جست وجو شاملspiritual care، cardiac disease، cardiac patient، heart disease، coronary artery disease و معادل فارسی آنها بودند. مقالاتی که جهت مرور انتخاب شدند درباره موضوع مشترک مراقبت معنوی و بیماران قلبی، به زبان فارسی یا انگلیسی و دارای متن کامل قابل دسترس بودند. یافته ها: در مجموع، 27 مقاله مورد بررسی قرار گرفتند. نیاز معنوی بیماران قلبی، تاثیر مداخلات معنوی در کاهش علایم بیماران قلبی و مشکلات رسیدن به این هدف و راه برطرف کردن آنها مرور و جمع بندی شد. نتیجه گیری: ارایه مراقبت معنوی به افراد مبتلا به بیماری های قلبی سبب بهبود کیفیت زندگی، بهزیستی معنوی، کاهش اضطراب و افسردگی می گردد؛ لذا ضروری است مراقبت معنوی در مراقبت های معمول افراد با بیماری قلبی گنجانده شود.

Abstract: 

Background- Valvular heart disease is one of the common complications of acute rheumatic fever, which manifests its clinical complications one or two decades after the primary attack of the disease in adults. The disease may involve one or more valves as stenosis or regurgitation or both and may cause congestive heart failure due to chronic volume or pressure overload.Method- This prospective study was performed from March 1999 to February 2001 in university affiliated medical centers. 53 patients (28 females and 25 males) who suffered from valvular heart disease in the form of mitral and aortic stenosis or regurgitation were studied. Most of the patients were in functional class I or II (44) and the rest (9) in class III and IV. Non-cardiac surgery procedures were laparotomy, orthopedic surgery, and thoracotomy.Results- Class I patients were able to easily tolerate the surgery without any significant problems. Only 2 patients had atrial fibrillation with rapid ventricular response in this class. In class II, one case had paroxysmal supraventricular tachycardia after thoracotomy and 3 patients had atrial fibrillation with rapid ventricular response. Another patient in this class had acute pulmonary edema. In class III, one patient had atrial fibrillation with rapid ventricular response after vaginal delivery. Of the two class IV patients, one patient needed reoperation due to prosthetic valve malfunction in the 26th week of pregnancy and the other had no significant hemodynamic problem.Discussion- Supraventricular arrhythmia, which occurs as atrial fibrillation or paroxysmal supraventricular tachycardia or sinus tachycardia mostly in the first 24 hours after surgery, is preventable with sufficient analgesic prescription. Careful monitoring is necessary in patients with severe mitral or aortic stenosis and double valve disease during surgery or the first 24 hours after surgery. As a result, the important factor in patient prognosis is the functional class of heart disease rather than the type of valvular disease
